Back to School 2004

Immunizations

Up-to-date immunizations are required for all new students as follows:

Grades K-2: Measles vaccine (second dose, preferably MMR); hepatitis B series (three doses over a six-month period); DTP (four or five doses required); polio (three or four doses required). Prekindergarten, kindergarten and first-grade students must have the varicella vaccine or documentation of past chicken pox.

Grades 3-6: Measles vaccine (second dose, preferably MMR); DTP (four or five doses required); polio (three or four doses required). Sixth-graders must have begun the hepatitis B series by October.

Grades 7-11: Tetanus-diphtheria (TD) booster; measles vaccine (second dose, preferably MMR); hepatitis B series (three doses given over a six-month period).

All immunization dates must be on the Florida Certificate of Immunization (DH 680). For immunization information, contact your private physician or the Hernando County Health Department.
